Why Specialized PT Matters

A new joint needs
a trained therapist

Total joint replacements are among the most common and successful procedures in orthopedic surgery — but the outcome depends heavily on what happens afterward. The implant is only part of the equation. The muscles, soft tissue, and movement patterns around the new joint determine your long-term function.

Generic PT that doesn't account for your specific implant type, surgical approach, and surgeon's protocol can set back your recovery or, in rare cases, risk the integrity of the new joint. At Cowboy Healthcare, we review your operative report and coordinate with your surgeon's office before your first session.

We know hip precautions for posterior vs. anterior approaches. We know the difference between cemented and cementless fixation timelines. And we advance you based on your actual tissue response — not a calendar on the wall.

Joint-Specific Programs

Each joint.
Its own protocol.

Total Knee Replacement

The most common total joint procedure. Our knee replacement program focuses on three critical outcomes: flexion range (goal: 120°+), quadriceps strength, and gait normalisation.

  • Swelling management and early range of motion
  • Quad sets and straight leg raises from day one
  • Gait training with and without assistive device
  • Stair negotiation and functional transfer training
  • Progressive strengthening to full weight-bearing

Total Hip Replacement

Hip precaution compliance is critical in the first 6–12 weeks. We incorporate your surgeon's specific precautions (posterior vs. anterior approach) into every session and home program.

  • Hip precaution education and home setup guidance
  • Early ambulation and weight-bearing progression
  • Abductor and hip flexor strengthening
  • Gait re-training to eliminate Trendelenburg pattern
  • Return to driving, stairs, and daily activities

Total Shoulder Replacement

Shoulder replacement rehab follows a strict passive-to-active progression to protect the repaired rotator cuff and allow subscapularis healing after a standard approach.

  • Passive range of motion within surgeon-specified limits
  • Sling weaning per protocol
  • Active-assisted and then active range of motion
  • Rotator cuff strengthening phase (typically weeks 6–12)
  • Functional overhead and behind-back goals

Partial & Revision Replacements

Partial (unicompartmental) knee replacements and revision procedures have different rehab timelines and considerations. We tailor the program to your specific implant and surgical findings.

  • Faster progression for partial replacements
  • Extended protection protocols for revision cases
  • Bone graft and augment healing considerations
  • Scar tissue management and manipulation if needed
  • Coordination with your revision surgeon throughout

Patient Reviews

What our patients
are saying

★★★★★

"Had my hip replaced in December. Started PT at Cowboy two weeks post-op and was walking without a walker by week five. The progressive strengthening program was exactly right — not too fast, not too slow."

William B. Hip Replacement Rehab · January 2026
★★★★★

"Second knee replacement — had the first one at a different PT clinic, no comparison. The functional approach here — actually preparing you for stairs, getting in and out of a car — is so much more practical than just doing leg presses."

Barbara L. Total Knee Replacement · March 2026
★★★★★

"My surgeon told me the speed of my recovery was exceptional. I attribute it entirely to starting pre-op PT and then continuing with Cowboy post-surgery. The progression was smart and the team communicated clearly with my surgical team."

Thomas S. Total Knee Replacement · February 2026
