Acceptance of Terms
By accessing the Cowboy Healthcare Physical Therapy website or receiving, scheduling, or inquiring about physical therapy services, you ("patient," "client," or "you") agree to be legally bound by these Terms and Conditions ("Terms"). These Terms form a binding agreement between you and Cowboy Healthcare, LLC ("Cowboy Healthcare," "we," "us," or "our"). If you do not agree to these Terms, do not use our website or services.
We may update these Terms at any time. The current version is always available on this page. Continued use of our services after any update constitutes acceptance of the revised Terms.
Eligibility and Patient Responsibility
Physical therapy services at Cowboy Healthcare are available to individuals of all ages. Minors must have parental or legal guardian consent before receiving services, and a parent or guardian must be present or available during treatment of patients under 18. By scheduling an appointment, you represent that you have the authority to consent to treatment for yourself or, where applicable, for the minor in your care.
Services Provided
Cowboy Healthcare, LLC provides outpatient physical therapy services in Ohio, delivered by licensed Doctors of Physical Therapy (DPT). Services include but are not limited to: musculoskeletal rehabilitation, sports injury recovery, post-surgical rehabilitation, home health physical therapy, dry needling, cupping, shockwave therapy, and therapeutic exercise programming.
All clinical services are provided at the professional judgment of our licensed providers. We reserve the right to decline or discontinue services where, in the clinical judgment of the treating therapist, treatment is not clinically appropriate or safe.
Appointments and Scheduling
Appointments may be scheduled through our online booking system, by phone at (330) 235-9142, or by referral from a physician or specialist. The following scheduling policies apply:
- Cancellation notice: We require at least 24 hours' notice to cancel or reschedule an appointment without penalty.
- Late cancellations (within 24 hours) and no-shows may be subject to a cancellation fee at the provider's discretion, and a pattern of late cancellations may result in discharge from care.
- Tardiness: Patients arriving more than 15 minutes late may need to reschedule their appointment to ensure adequate treatment time for all patients.
- Provider cancellations: We reserve the right to cancel or reschedule appointments due to provider illness, emergency, or unforeseen circumstances. We will notify patients as promptly as possible and offer the earliest available alternative time.
- Telehealth: Where available, telehealth services are subject to the same scheduling and cancellation policies as in-person appointments.
Medical Disclaimer
Information provided on our website — including descriptions of conditions, treatment methods, and expected outcomes — is intended for general informational purposes only and does not constitute medical advice, diagnosis, or treatment. No content on this site should be used as a substitute for professional medical advice from a licensed healthcare provider who knows your individual circumstances.
Always consult a qualified physician or healthcare professional before beginning any physical therapy, exercise, or rehabilitation program. Cowboy Healthcare is not liable for any adverse outcome resulting from self-directed activity, independent interpretation of website content, or decisions made without consulting a licensed provider.
Physical Therapy Treatment Consent
By proceeding with physical therapy services, you consent to evaluation and treatment by our licensed Doctors of Physical Therapy. You acknowledge that:
- Physical therapy involves hands-on manual therapy, therapeutic exercise, and other interventions that carry inherent physical risks, including temporary soreness or discomfort.
- You have disclosed all known medical conditions, medications, surgeries, and relevant health history to your treating therapist.
- You may withdraw consent to any specific treatment or technique at any time during your care without affecting your right to continue other aspects of treatment.
- A formal consent form will be provided and signed at your initial evaluation, which governs the clinical aspects of your care. These Terms govern the business and contractual relationship between you and Cowboy Healthcare.
Payment and Billing
Payment is due at or before the time of each service unless a specific payment arrangement has been established in writing with our billing department. We accept cash, major credit and debit cards, and select insurance plans. The following billing terms apply:
- Patients are responsible for all co-pays, co-insurance, and deductibles at the time of service.
- Any balance not covered by insurance — including denied claims — remains the financial responsibility of the patient.
- Unpaid balances may be referred to a collection agency after 90 days with prior written notice to the patient.
- We will provide a Good Faith Estimate of expected costs upon request, in compliance with the No Surprises Act where applicable.
Billing questions: info@cowboyhealthcare.com or (330) 235-9142.
Insurance
Cowboy Healthcare accepts select insurance plans including Medicare, Medicaid (where applicable), BCBS, UnitedHealthcare, Aetna, Cigna, Humana, GEHA, MultiPlan, and VA/Tricare. Insurance acceptance is subject to change.
It is the patient's responsibility to:
- Verify that Cowboy Healthcare is an in-network provider with their specific plan before scheduling.
- Obtain any required referrals, authorizations, or pre-certifications from their insurer prior to beginning treatment.
- Provide accurate and current insurance information at each visit.
- Notify us of any changes to insurance coverage as soon as possible.
Cowboy Healthcare does not guarantee coverage, reimbursement, or authorization for any specific service under any specific insurance plan. Any amount not covered by insurance is the patient's responsibility.
Medicare Compliance
For Medicare beneficiaries, Cowboy Healthcare complies with all applicable Medicare billing regulations. An Advance Beneficiary Notice (ABN) will be provided before any service that we anticipate Medicare may deny as non-covered or not medically necessary, allowing you to make an informed decision before proceeding.
HIPAA and Health Information
Your health information is protected under the Health Insurance Portability and Accountability Act (HIPAA). A copy of our HIPAA Notice of Privacy Practices is provided at your first appointment and available upon request. We will not share your protected health information without your written authorization except as permitted under HIPAA (e.g., treatment, payment, healthcare operations, and legally required disclosures). See our full Privacy Policy for details.
Refund Policy
- Pre-paid service packages are non-refundable once treatment has commenced, except as required by applicable law.
- Overpayments will be refunded to the original payment method within 30 days of identification.
- Refund requests for cancelled pre-paid packages (prior to any treatment) will be reviewed on a case-by-case basis.
Patient Conduct
We are committed to providing a safe, professional, and respectful environment for all patients and staff. We reserve the right to discharge any patient from care if their behavior is abusive, threatening, discriminatory, or otherwise disruptive to staff or other patients. Discharge for conduct reasons does not entitle the patient to a refund for services already rendered.
Photography and Recording
Photography, video, or audio recording of clinical sessions is not permitted without prior written consent from the treating therapist. Cowboy Healthcare may photograph or record clinical demonstrations or facility images for educational or marketing purposes; affected patients will be asked for written consent in advance.

Our total aggregate liability for any claim shall not exceed the total fees paid by the patient in the three (3) months prior to the event giving rise to the claim.
Nothing in these Terms limits liability for gross negligence, willful misconduct, or claims that cannot be limited under applicable law.
Indemnification
You agree to indemnify, defend, and hold harmless Cowboy Healthcare, LLC and its officers, directors, employees, agents, and affiliates from any claims, damages, losses, liabilities, costs, and expenses (including reasonable legal fees) arising out of your use of our services, your violation of these Terms, or your violation of any applicable law or third-party right.
Governing Law and Dispute Resolution

Severability
If any provision of these Terms is found invalid or unenforceable under applicable law, it shall be modified to the minimum extent necessary to make it enforceable. All remaining provisions shall remain in full force and effect.
Entire Agreement
