Acceptance of Terms

By accessing the Cowboy Healthcare Physical Therapy website or receiving, scheduling, or inquiring about physical therapy services, you ("patient," "client," or "you") agree to be legally bound by these Terms and Conditions ("Terms"). These Terms form a binding agreement between you and Cowboy Healthcare, LLC ("Cowboy Healthcare," "we," "us," or "our"). If you do not agree to these Terms, do not use our website or services.

We may update these Terms at any time. The current version is always available on this page. Continued use of our services after any update constitutes acceptance of the revised Terms.

Eligibility and Patient Responsibility

Physical therapy services at Cowboy Healthcare are available to individuals of all ages. Minors must have parental or legal guardian consent before receiving services, and a parent or guardian must be present or available during treatment of patients under 18. By scheduling an appointment, you represent that you have the authority to consent to treatment for yourself or, where applicable, for the minor in your care.

Services Provided

Cowboy Healthcare, LLC provides outpatient physical therapy services in Ohio, delivered by licensed Doctors of Physical Therapy (DPT). Services include but are not limited to: musculoskeletal rehabilitation, sports injury recovery, post-surgical rehabilitation, home health physical therapy, dry needling, cupping, shockwave therapy, and therapeutic exercise programming.

All clinical services are provided at the professional judgment of our licensed providers. We reserve the right to decline or discontinue services where, in the clinical judgment of the treating therapist, treatment is not clinically appropriate or safe.

Physical Therapy Treatment Consent

By proceeding with physical therapy services, you consent to evaluation and treatment by our licensed Doctors of Physical Therapy. You acknowledge that:

Payment and Billing

Payment is due at or before the time of each service unless a specific payment arrangement has been established in writing with our billing department. We accept cash, major credit and debit cards, and select insurance plans. The following billing terms apply:

Insurance

Cowboy Healthcare accepts select insurance plans including Medicare, Medicaid (where applicable), BCBS, UnitedHealthcare, Aetna, Cigna, Humana, GEHA, MultiPlan, and VA/Tricare. Insurance acceptance is subject to change.

Cowboy Healthcare does not guarantee coverage, reimbursement, or authorization for any specific service under any specific insurance plan. Any amount not covered by insurance is the patient's responsibility.

Medicare Compliance

For Medicare beneficiaries, Cowboy Healthcare complies with all applicable Medicare billing regulations. An Advance Beneficiary Notice (ABN) will be provided before any service that we anticipate Medicare may deny as non-covered or not medically necessary, allowing you to make an informed decision before proceeding.

HIPAA and Health Information

Your health information is protected under the Health Insurance Portability and Accountability Act (HIPAA). A copy of our HIPAA Notice of Privacy Practices is provided at your first appointment and available upon request. We will not share your protected health information without your written authorization except as permitted under HIPAA (e.g., treatment, payment, healthcare operations, and legally required disclosures). Refund Policy

Patient Conduct

We are committed to providing a safe, professional, and respectful environment for all patients and staff. We reserve the right to discharge any patient from care if their behavior is abusive, threatening, discriminatory, or otherwise disruptive to staff or other patients. Discharge for conduct reasons does not entitle the patient to a refund for services already rendered.

Photography and Recording

Photography, video, or audio recording of clinical sessions is not permitted without prior written consent from the treating therapist. Cowboy Healthcare may photograph or record clinical demonstrations or facility images for educational or marketing purposes; affected patients will be asked for written consent in advance.

Intellectual Property

All content on this website — including text, clinical descriptions, images, logos, and design — is the intellectual property of Cowboy Healthcare, LLC and may not be reproduced, copied, distributed, or used in any commercial context without prior written permission.

Limitation of Liability

To the fullest extent permitted by applicable law, Cowboy Healthcare, LLC, its physicians, therapists, employees, agents, and affiliates shall not be liable for any indirect, incidental, punitive, special, or consequential damages arising from the use of our services or website, including but not limited to loss of income, loss of data, personal injury not caused by our negligence, or treatment outcomes. Our total aggregate liability for any claim shall not exceed the total fees paid by the patient in the three (3) months prior to the event giving rise to the claim.

Nothing in these Terms limits liability for gross negligence, willful misconduct, or claims that cannot be limited under applicable law.

Indemnification

You agree to indemnify, defend, and hold harmless Cowboy Healthcare, LLC and its officers, directors, employees, agents, and affiliates from any claims, damages, losses, liabilities, costs, and expenses (including reasonable legal fees) arising out of your use of our services, your violation of these Terms, or your violation of any applicable law or third-party right.

Governing Law and Dispute Resolution

These Terms are governed by the laws of the State of Ohio without regard to its conflict of law principles. Any dispute arising from or relating to these Terms or our services shall be resolved exclusively in the state or federal courts of Portage County, Ohio, and you consent to personal jurisdiction there.

Before initiating formal legal proceedings, both parties agree to attempt to resolve disputes informally in good faith. Please contact us at info@cowboyhealthcare.com; we will respond within 30 days.

Severability

If any provision of these Terms is found invalid or unenforceable under applicable law, it shall be modified to the minimum extent necessary to make it enforceable. All remaining provisions shall remain in full force and effect.

Entire Agreement

These Terms, together with our Privacy Policy and any signed clinical consent forms, constitute the entire agreement between you and Cowboy Healthcare, LLC regarding physical therapy services and supersede all prior agreements, representations, and understandings.
